Annual Report as Proof of Not-for-Profit Status

For most Not-for-Profit Organizations, the Annual Report to the Secretary of State is the correct not-for-profit verification. This form must be filed each year to maintain an organization's not-for-profit status. The IAC will accept a copy of the Annual Report AND a copy of the cancelled check (front and back) that accompanied the Annual Report. To be valid, the Annual Report and check must be dated no more than one year prior to the application date. Submit a PDF of these documents electronically through the Illinois eGrant system.
