The IAS application consists of electronic and non-electronic components. Each component must be completed in full and submitted in the format outlined in these guidelines by the application deadline.
Electronic Components:
The electronic portion of the application is found on the eGrant website and consists of form fields and the uploading of required documents.

The electronic portion of the application must be successfully submitted to Illinois eGrant no later than 5:00 pm, Friday, September 14, 2012.
Non-Electronic Components:
In addition to the application submitted through eGrant, applicants must submit the following required materials to the Illinois Arts Council by mail or hand-delivery:

Work Sample(s) in the number and format specified for the artistic discipline (see work sample section)

One copy of the Work Sample Inventory

Self-addressed stamped (not metered) envelope of appropriate size and protection (if applicant wishes samples to be returned).
The required materials should be clearly marked with your first and last name and the egrant application ID number which can be found on the eGrant menu below the name of the application you are working on (ID: xxxxxx)
The required materials must be postmarked or hand-delivered no later than Friday, September 14, 2012. Hand-delivered materials must be received by 4:30 pm.
Required Attachments
The attachments outlined below are required for all applicants and must be uploaded as part of the eGrant application. Read directions carefully, IAS applications submitted without the required attachments outlined below will be ruled ineligible.
All attachments for which a template is not provided must:

Not be handwritten
-
Have at least a 1” margin; use black, 12-point font or a larger type size

Have your name on the top right corner of each page

Have sequentially numbered pages

Not be typed in all capital letters

Not be photo reduced

All required documents must be saved as PDF files.
Each document should be labeled as follows: Lastname_Firstname_Attachment Letter.pdf . Example:

Save all required attachments to your computer’s desktop in PDF format and labeled as outlined in these guidelines. Upload all required attachments to your eGrant application from your computer’s desktop.
If you do not have software to convert documents to PDF files, listed below are three PDF converter options:
Attachment A: Project Narrative –
There are different questions and page requirements for Track 1: IA-PD and Track2: IA-Project .
Project Narrative for Track1: IA-PD applicants only.
Address the following four areas in no more than one page:

Provide the relevant facts of your professional development opportunity (e.g. attend writer’s workshop, hire a web designer, etc).

Describe the unique and significant gain this opportunity will provide for your work and your career.

Describe the specific expenses you intend to pay for using this grant (e.g. conference fee, professional design fee, etc.)
-
Include a timeline and/or date of the professional development activity.
-
If applicable, provide website links for workshops, artists' retreats, conferences or symposia for which you are applying for support to attend.
Address the following four areas in no more than two pages:

Project Description

Briefly describe the activity to be funded by this grant. Include a description of the proposed public release, presentation, performance, or exhibition of the completed activity.

Describe the specific type(s) of expenses you propose to pay with IAC grant funds, e.g. materials and supplies costs, pay dancers, rent performance space, hire marketing/pr firm.

Describe who is responsible for carrying out the proposed activities. Include the role of the applicant and the names, backgrounds, and roles of any other creative artist, organization(s), or representative(s) of organization(s) whose involvement is required to successfully carry out the proposed activity.

Describe why this activity is important to you/your career at this time.

Describe the target audience for your activity and the type of presentation/release that will be made during the grant period.

Attachment B - Project Budget
Complete a budget for the proposed activity outlined in Attachment A using the template provided. The budget consists of Project Expenses and Project Income. For Examples of project budgets, refer to the IAS Project Budget Templates page.

EXPENSES -Itemize a brief list of expenses for the activity that this grant will cover. Be specific about items, amounts, and note whether the expense is actual or anticipated.

INCOME - Itemize your sources of income both cash and in-kind. Indicated if the source is Confirmed (C) or Anticipated (A). *
Note: Your project income should be equal to your project expenses.
*Applications for the Artist Project Track (IA-Project) require a minimum of 25% cash match. The cash match required is as follows:

$1,500 request = $ 375 cash match required

$2,500 request = $ 625 cash match required

Attachment C - Artistic Resume/Bio -
Submit an artistic resume or bio of no more than two pages. Academic resumes are not encouraged.

Attachment D - Artist Statement -
Submit an artist statement of no more than one page (single sided).
Applicants to all disciplines, with the exception of Ethnic and Folk Arts should use this statement to describe:

your evolution as an artist,
-
what challenges you currently face in your career/work, and
-
what your plans are for the future.
Ethnic and Folk Arts applicants must use this statement to describe:
-
the traditional art form you practice and how you learned it, from whom, when, and where you learned it,
-
how the art form and your experience express the culture and communities from which it springs, and
-
how the practice of this traditional art enriches your life and the life of your community.

what challenges you currently face in your career/work, and
-
what your plans are for the future.

Attachment E - Work Sample Inventory (for all disciplines with the exception of Literature and Theatre) -

Submit a work sample inventory of no more than one page (single sided).

Label the Work Sample Inventory in the upper right-hand corner with your first and last name.

If the production of the work involves others, explain your role.
-
If you are performing with others, describe yourself to help the panel identify you. For example, "I am the singer in the green costume" or I am playing the middle viola".

DO NOT attach additonal pages to further describe your work.

In addition to uploading this attachment, you must sbumit a hard copy with your work samples.

If you feel your work cannot be represented or described in the manner outlined, contact the IAC staff to discuss alternatives

If submitting a work sample on DVD or CD - indicate the following:

Attachment E - Work Sample Inventory and Work Sample for Literature and Theatre ONLY -

submit a maximum of eleven (11) pages. The first page must be the work sample inventory listing followed by up to10 pages of a typed manuscript .

each page should have your first and last name and “Attachment E” typed in he top right-hand corner,
manuscripts must be typed, double-spaced, with one-inch margins of all sides*,
font size must be 12 pt. or larger,
for Poetry work samples, include no more than one poem per page.
* Poetry submissions may be single-spaced only if double-spacing would impinge on the interpretation or format of the work.
For Poetry submissions, the inventory/first page of the manuscript should indicate the following for each piece submitted in the order it appears in the manuscript:

title of poem and number of pages
-
the date completed, or if work-in-progress, anticipated date of completion.
For Prose submissions (fiction, creative non-fiction), the inventory/first page of the manuscript should indicate the following for each piece submitted in the order it appears in the manuscript:

title of work,

genre,
-
number of pages being submitted,
-
date completed, or it work-in-progress, anticipated date of completion, and
-
if the manuscript is an excerpt from a larger work , a brief synopsis of the work, including the submitted excerpt and its relationship to the entire work.
For Theatre submissions (stage/screen/tele/plays, librettos, etc.), the first page of the manuscript should indicate the following:

title of work,
-
title of excerpted scene or act submitted

genre (e.g. stage play, screenplay, teleplay, libretto, radio play, audio drama, etc),
-
total length in pages,
-
date completed, or if work in progress, anticipated date of completion, and
-
a brief synopsis of the script including the submitted excerpt and its relationship to the entire work
NOTE:

Do not send additional manuscript pages. Manuscripts containing more than the maximum allowable number will be disqualified immediately.

Photocopies of publications will not be accepted and will immediately disqualify the applicant.
Manuscript (including work sample inventory listing) should be saved as a PDF file and labeled as follows: lastname_firstname_E.
Attachment F - Proof of Illinois Residency
Applicants must upload a copy of one of the following items at the time of application:

Illinois driver’s license which includes the date issued and expiration date (photocopy entire front and back);
-
State of Illinois identification card which includes the date issued and expiration date; (photocopy entire front and back);
-
Illinois state income tax forms for the most recent year;
-
Illinois Voter Registration card
NOTE: Attachment F must be submitted electronically by uploading the document to the Illinois eGrant application.
