Samples of work completed within the past four years (2001
or later), in the format and number requested, must accompany the application
form unless special arrangements are made with the Supervising Director for
Programs. If you feel your work cannot be adequately represented in one of the
formats described, contact the Supervising Director for Programs to discuss
alternatives.
NOTE: Past Fellowship Recipients must apply with new work.
IAC staff makes every effort to handle work samples with
care; however, the IAC cannot be held responsible for any damage or loss that
may occur. Applicants are urged to submit copies of original
tapes, manuscripts, scores, films, etc - not the original work.
RETURN/RETENTION OF WORK SAMPLES
Applicants who wish their work samples returned to them
by mail must enclose a self-addressed stamped (not metered) envelope of appropriate
size and protection.
Due to the lack of storage space, unclaimed work
samples will be destroyed 60 days after Fellowship Awards are announced.
PROSE SCRIPTWORKS MANUSCRIPTS WILL NOT BE RETURNED.
Work samples of Fellowship or Finalist Award recipients
will be retained or copied by the Illinois Arts Council for its archives or
submission to a public-use archive. By submitting work samples, applicants give
permission to the IAC to use images and content in IAC produced publications.

including your role and a brief description of the work submitted including
the cued segment (if applicable) and its relationship to the entire piece.
NOTE: During the initial review, the jury will view 2 to 5
minutes of your first selection and up to 5 minutes of your second selection.
For those applicants selected for the review's final round, the jury will view
no more than 10 minutes of each selection. You are urged to submit entire works
as the jury may elect to view different parts of the work submitted. It is preferable
that the camera remain in one location during the taping. If the performance
space is large, the camera may pan and/or focus in on smaller more detailed
movements, while keeping the performer in full view. Tapes should be within
the framework of what is possible in a live performance; special effects, dissolves,
super-impositions, etc., are highly discouraged.
For individual applicants: The first selection
submitted for review must be of a non-collaborative work conceived solely by
the applicant. The second selection may be of a collaborative work. If a collaborative
work is submitted, an explanation of your role in the conception, creation and
production of the work must be outlined in the Work Sample Sheet.

are encouraged to show at least five different works.

NOTE: Slides are reviewed three at a time.
Slides #1, #2 and #3 are projected simultaneously, then #4, #5 and #6 and lastly
#7, #8 and #9. If you plan to show details or different views of a work, your
slides should be ordered in such a way that related slides are projected together.
NOTE: You are strongly urged to submit the maximum number
of slides requested for a more consistent, quality presentation. Please be aware
that the award decisions are based heavily on the work samples submitted. Therefore,
it is of the utmost importance to submit the highest quality slides.
All Ethnic and Folk Arts Applicants must submit 10 copies
of a narrative of no more than two pages which responds to the following questions.
(Within the narrative, you may also include other information which you feel
is significant.)
Describe the traditional art you practice and how
you learned it.
When, where, and from whom did you learn the traditional
art form you practice?
How does the art form relate to your heritage?
How does this art form function in your community?
How does this art form and your experience express
the ethnic, language, regional, tribal,
or nationality group from which it springs?
How does the practice of this traditional art enrich
your life and the life of your community?
WORK SAMPLE INSTRUCTIONS
MATERIAL CULTURE
Submit a minimum of six and a maximum of nine 35mm
slides or photographs. (Slides are strongly recommended.) Include samples
of at least four objects. One close-up/detail is helpful in determining artistic
excellence.

disc (CD), DVD or VHS videotape. Applicants should select the audiovisual
format which best demonstrates their ability. VHS or DVD are highly recommended
for Storytelling applicants;

Master dance instructors should submit samples that
demonstrate their teaching ability, their students' achievements and their
own dance mastery.

If text is not in English, 10
copies of an English translation must be provided.

applicants selected for the review's final round, the jury will view no more

Pages must be numbered consecutively in the upper
right-hand corner and titles of pieces should appear at the top of every page.
Each copy should include a detachable cover sheet
stating your name and address. Your name must not appear on other pages as
the initial review is anonymous.
Do not attach application materials to manuscripts.
Books, magazines and photocopies thereof will not
be accepted.
Manuscripts must be entirely fiction or creative
non-fiction, not a combination.
Pages in excess of the maximum will not be accepted.
Writers who submit a section of a longer piece must
limit their submission to 30 pages. It should be noted, however, that the
selection should be in sequential order and be able to stand on its own as
an autonomous, coherent piece of prose.
Work Sample Sheet
On a sheet of paper, indicate the following for each piece
submitted in the order it appears: your name, title of work, genre, number of
pages and date completed.
Applicants must submit five copies of one complete original
script that was completed within the past four years (1999 or later).
NOTE: Copies of scripts will not be returned. Be sure to
keep a copy of what you send.
